Duties
• Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
• Monitor patients’ health by taking vital signs, observing for changes, and reporting findings to healthcare professionals
• Prepare nutritious meals tailored to individual dietary needs and assist with feeding when required
• Administer medications accurately following care plans and HIPAA regulations to ensure patient safety
• Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing personal spaces
• Support clients during outings or community activities, promoting social engagement and independence
• Document care activities meticulously in compliance with healthcare standards and care plans
Requirements
• Proven experience in home & community care, assisted living, nursing homes, or group homes; experience working with individuals with developmental disabilities, autism, dementia (including Alzheimer’s), or memory care is highly valued
• Certification in First Aid and CPR; knowledge of HIPAA regulations for patient privacy and confidentiality
• Ability to perform heavy lifting safely and operate mobility aids such as Hoyer lifts or resident lifts
• Strong observational skills for patient monitoring and recognizing signs of distress or health deterioration
• Familiarity with medication administration, vital signs measurement, and care plan adherence
• Excellent communication skills to collaborate effectively with clients, families, and healthcare teams
• Flexibility to work various shifts including weekends or evenings as needed; compassionate attitude and patience are essential
